Magnesium acts as a vital cofactor for multiple enzymes and helps regulate calcium and potassium levels, while phosphate is integral to various metabolic and signaling cellular processes.

Depletion of these elements correlates with multiple symptoms and adverse outcomes. The rapid identification, diagnosis, and management from internists are crucial for optimal results across different medical settings.

Pearl 1: When is Magnesium indicated and what are the major considerations to keep in mind?

  1. Magnesium's allure stems from its potential benefits, capturing interest in medical circles!
    1. Established uses:
      1. Torsades
      2. Eclampsia
    2. Potential uses (being investigated, mostly associations and limited evidence):
      1. Asthma Exacerbation
      2. Migraines
      3. Sleep Quality
      4. Cataracts
      5. Depression
    3. However, consider in Gitelman syndrome
      1. Patients with chronically low magnesium WITHOUT higher rates of migraines or asthma
  2. Magnesium is excreted in urine and feces
    1. Requires cautious administration in patients with kidney dysfunction
  3. Obstetrics Patients vs. Medicine Patients
    1. Obstetrics Patients
      1. Normal Kidney Function
        1. Supratherapeutic magnesium levels in obstetrics protect against seizures
    2. Medicine Patients
        1. Impaired kidney function can lead to high magnesium levels (above >4 meq/L), which can cause:
          1. Neuromuscular issues
          2. Weakness
          3. Confusion
  4. Data favors magnesium repletion among electrolytes
    1. Potential benefits in acute MI!
      1. Studies supporting benefit
        1. A 1995 study supports IV magnesium for better outcomes in acute MI cases
      2. Studies showing inconsistent benefit
        1. ISIS-4 study
          1. Control group mortality rate was 7.2% (significantly lower than previous trials)
            1. Raising the possibility that more ISIS-4 patients were in a low-risk group unlikely to benefit from magnesium
        2.  MAGIC study
    2. Magnesium's strongest data lies in heart failure and post-MI care!
      1. BUT potential associations exist between magnesium repletion and morbidity/mortality benefits in many other areas
  5. Magnesium's impact varies, suggesting a cautious approachneither highly effective nor harmful.
    1. However, some evidence indicates a potential for being optimistic
      1. Mortality with hypomagnesemia
        1. Hypomagnesemia is with higher mortality in critically ill patients
      2. Atrial Fibrillation
        1. A 2007 meta-analysis supports magnesium's role in rapid atrial fibrillation control.
      3. Hyperkalemia
        1. Magnesium positively affects potassium levels, helping manage hypokalemia-induced hyperkalemia.

Pearl 2: Common Etiologies of hypomagnesemia

  1. GI Losses/Impaired Absorption (1/3rd of Mg absorption through GI tract)
    1. Proton-pump inhibitors
      1.  Altering gastric pH and affects magnesium absorption
      2. Take a look at the where you see PPI stopped  magnesium goes up, PPI reinstituted, it plummets .
    2. Acute or chronic diarrhea
    3. Malabsorption
    4. Small bowel bypass surgery
    5. Pancreatitis
      1. Similar to hypocalcemia, Mag and Ca+2 can get deposited in areas of (saponification)
  2. Urinary loss aka Renal magnesium wasting
    1. Medications
      1. Diuretics
        1. Thiazide
        2. Loop
      2. Antibiotics
        1. Aminoglycosides
        2. Amphotericin B
        3. Foscarnet
      3. Chemotherapeutic agents
        1. Cisplatin
        2. EGFR inhibitors
      4. Immunosuppressive agents
        1. Calcineurin inhibitors
          1. Tacrolimus
          2. Cyclosporine
        2. mTor inhibitors
      5. NOTE: Addressing the root cause, such as discontinuing PPIs ,can be crucial for effective magnesium management!
    2. Osmotic diuresis in the setting of elevated blood sugars
    3. Alcohol use disorder
      1. Coincides with limited oral intake of Mg
    4. Hypercalcemia
    5. Genetic Tubular Disorders
    6. Other Acquired Tubular Dysfunction
      1. Post-obstructive diuresis 
      2. Osmotic diuresis s/p kidney transplant
  3. If the cause is not obvious, urinary magnesium levels can indicate whether urine (kidney) or stool (GI tract) is the source of magnesium wasting
    1. Renal Magnesium Wasting:
      1. 24 hour urinary magnesium >10-30 mg OR
      2. Fractional excretion of magnesium (FEMg) >3-4%
        1. If FEMg <1% probably non-renal causes!

Pearl 3: Correcting hypomagnesemia and magnesium repletion

  1. How to correct hypomagnesemia?
    1. Limit losses:
      1. Address correctable causes (PPI, kidney function, etc)
    2. Increase intake:
      1. Consider supplementing with magnesium-rich foods
        1. Nuts, legumes, pumpkin seeds, leafy vegetables, chocolate, lima beans, and avocados
  2. Magnesium repletion
    1. IV administration
      1. Can lead to transient increases in serum magnesium levels
        1. Should be administered over extended periods to prevent rapid renal excretion!
        2. Loop of Henle largely reabsorbs magnesium, influencing transient serum increase
    2. Oral administration (PO magnesium oxide)
      1. Can cause diarrhea
        1. May be preferred in patients with constipation
          1. Dual-purpose regimen: magnesium repletion + address constipation
    3. Unlike in potassium supplementation, there is not a good rule of thumb to predict how repletion with a certain amount of PO magnesium (in milligrams) translates to an increase in serum magnesium level (in milligrams per deciliter).

Pearl 4: Phosphorus

  1. What causes hypophosphatemia?
    1. Low intake
      1. Not as common
        1. Phosphate is plentiful in foods
        2. Kidneys can limit phosphate loss to 0%
    2. GI losses
      1. Any malabsorption or diarrhea
      2. Common OTC antacids with aluminum or magnesium
        1. Binds to phosphate insoluble salts pooped out
    3. Kidney loss
      1. Mostly governed by hormones
        1. Vitamin D deficiency
        2. Secondary hyperparathyroidism
      2. Diuretics 
        1. Most phosphate reabsorption happens in proximal tubules so makes sense the ones that act more proximally will affect phosphate level
          1. Acetazolamide
          2. Metolazone
            1. Has some carbonic anhydrase inhibitory properties
    4. Cellular shifts
      1. DKA
      2. Refeeding syndrome
      3. Respiratory alkalosis
      4. Hungry bone syndrome
  2. What could happen to a patient with severe hypophosphatemia?
    1. Rhabdomyolysis and diaphragmatic weakness!
      1. < 1 millimole per liter concerning
      2. < 2 millimole per liter aggressive repletion needed!
  3. How to replenish phosphorus?
    1. What to use?
      1. Neutra-phos packets
        1. Contain 8 millimoles of phosphate + 7 millimoles of sodium and potassium
          1. Consider the patients potassium levels when administering phosphate-containing supplements to avoid excessive intake
          2. Make sure you understand how much sodium and potassium youre giving your patient!
      2. Phosphorus-rich foods examples:
        1. Milk
          1. Contains about as much phosphorus as two packets of neutra-phos!
        2. Eggs
        3. Nut butters
          1. Protein drinks
      3. IV Phosphate
        1. Be mindful of the amount of fluid given in our fluid sensitive patients
    2. How much to use?
      1. Algorithms suggest 40-80 millimoles of phosphate repletion for asymptomatic patients with low phosphate levels
        1. The volume of distribution of phosphate varies among patients, making repletion challenging.
