Lessons from the Transforming Clinical Practice Initiative (TCPi)

The Transforming Clinical Practice Initiative (TCPi) was launched by CMS in 2015 to help clinicians get ready for value-based payment by providing free tools, resources and coaching to improve patient outcomes and reduce healthcare costs. ACP was a TCPi grantee.

These resources and inspirational videos were developed as part of the TCPi to help practices learn from others about quality improvement efforts to create a more effective, efficient and patient-centered healthcare system.

TCPi Power Packs

TCPi Power Packs were developed by ACP and other grantees with input from CMS to address specific challenges that a practice might face as it begins the process of clinical practice transformation.
